Piercing Bumps Explained

What piercing bumps actually are, why they form and how to make them go away. The four types most clients encounter and the specific approach for each.

In short

Most piercing bumps are irritation hypertrophy, a localised tissue response to ongoing low-level disturbance. They look alarming but are almost always fixable by removing the cause (cheap jewellery, sleep pressure, touching, harsh products). True rejection, keloid scarring and abscesses are different and less common.

If you have a bump, do not panic. Identify the cause, remove it and the bump usually fades within a few weeks.

The piercing bump is the most common worry that brings clients back to the studio. The bump looks alarming, the internet predicts the worst and the panic level rises quickly. The honest reality is that almost all bumps are minor and fixable. This page explains what bumps actually are, what causes them and how to deal with each type.

The Four Types of Piercing Bump

Most “piercing bumps” fall into one of four categories. Telling them apart helps identify the right response.

1. Irritation Hypertrophy (The Common One)

A small bump of accumulated tissue and fluid near the piercing, caused by ongoing low-level disturbance. Skin-coloured or slightly red. Soft when pressed. May be slightly tender. Fades when the cause is removed.

2. Hypertrophic Scar

A raised firm scar at the piercing entry or exit, staying within the boundary of the original injury. Pink or red. Often shiny. Can be itchy. More persistent than irritation hypertrophy.

3. Keloid Scar

A raised firm scar that extends beyond the boundary of the original injury. Pink, red or darker than surrounding skin. Can grow over months. Genuine keloids are uncommon and have a strong genetic component.

4. Abscess

A pocket of pus, hot and tender, often with a soft fluctuant centre. Significantly more serious. Needs medical attention rather than studio attention.

Common

Irritation Bumps

Soft, skin-coloured or slightly red. Caused by ongoing irritation. Fade within weeks when the cause is removed.

The vast majority of “bumps” people worry about.

Rare

Other Types

Hypertrophic scars persist longer and need different treatment. Keloids have genetic component. Abscesses need medical care.

All less common than irritation bumps.

Irritation Hypertrophy: The Common Bump

What It Looks Like

A small raised area near the piercing entry or exit, sometimes both sides. Usually 2 to 5 millimetres in size. Skin-coloured or slightly pink. Soft when gently pressed. May produce occasional clear fluid.

What Causes It

The piercing is being disturbed at a low level on an ongoing basis, and the tissue is responding by accumulating around the irritation. Common causes:

  • Cheap or plated jewellery (the most common cause by far)
  • Sleeping on the piercing
  • Knocking it on clothing, hair or hands repeatedly
  • Touching it frequently
  • Twisting the jewellery
  • The wrong gauge or length of bar for the piercing
  • Harsh aftercare products
  • Over-cleaning

How to Fix It

Identify and remove the cause. The bump fades within 2 to 4 weeks once the underlying irritation is gone.

Specific Actions

  • Swap to implant grade titanium if wearing fashion jewellery
  • Set up sleep position to avoid pressure on the piercing
  • Stop touching and twisting
  • Simplify aftercare to just sterile saline
  • Come into the studio for a jewellery check if you are unsure of the cause

What Not to Do

Do not apply tea tree oil, vinegar, aspirin paste or other home remedies. They make the bump worse rather than better. Do not try to “drain” the bump with a needle. Do not panic and remove the jewellery.

Hypertrophic Scars

What It Looks Like

A raised, firm scar tissue formation right at the piercing entry or exit. Pink, red or slightly darker than surrounding skin. May be shiny. Stays within the boundary of the original piercing wound. Can be itchy.

What Causes It

The body’s healing response has been more aggressive than necessary, laying down excess collagen at the wound site. Often follows from prolonged irritation or repeated minor trauma. Can also occur in people who tend to scar this way generally.

How to Fix It

Address any ongoing irritation first (jewellery, sleep, products). The scar tissue itself can sometimes be reduced with specific treatments:

  • Silicone gel sheets or silicone gel applied to the scar for several weeks
  • Massage with mild moisturiser once the piercing is fully healed
  • Specific scar treatments from a GP or dermatologist for persistent cases
  • Patience: hypertrophic scars often soften and fade over months once the irritation is removed

The Difference From Irritation Bumps

Hypertrophic scars are firmer, more persistent and continue even when the cause has been removed. They take longer to fade and may need active treatment.

Keloid Scars

What It Looks Like

A raised, firm scar that extends beyond the original wound boundary. Pink, red or darker than surrounding skin. Can grow over months or years. May continue to enlarge even when no further irritation is present.

What Causes It

A genetic predisposition that causes the body’s scar-forming response to overshoot dramatically. Keloids have a strong family history component and are more common in people of African, Asian and Hispanic descent.

How to Identify If You Are Prone

If close family members have keloid scars from minor wounds, you may be predisposed. If you have keloid scars from previous piercings, surgery or minor injuries, you definitely are. Mention this to the piercer before booking; some piercings have higher keloid risk and may be worth avoiding.

How to Treat Them

Keloid treatment is medical rather than studio care. Options include:

  • Silicone gel sheets applied long-term
  • Steroid injections (most common medical treatment)
  • Cryotherapy (freezing)
  • Laser treatment
  • Surgical removal (controversial because keloids often return)

For active keloid management, see a GP for referral to a dermatologist.

Prevention

If you are keloid-prone, choose piercings with lower keloid risk (lobes are lower risk than helix or other cartilage), use the highest quality jewellery, follow aftercare meticulously and consider silicone gel sheets prophylactically once the piercing is healed.

Abscesses

What It Looks Like

A hot, tender, fluctuant (soft-centred when pressed) collection of pus. Skin over the abscess is often shiny and red. May discharge thick yellow or green pus. Pain is usually significant and increases over time. Spreading redness or fever indicate the infection is escalating.

What Causes It

Bacterial infection that has progressed beyond simple irritation. The body has walled off the infection into a pus-filled pocket.

How to Handle It

This is medical, not studio territory. See a GP same day if possible, A&E if symptoms are severe. Treatment usually involves antibiotics and sometimes drainage of the abscess. Do not try to drain it yourself.

Do Not Remove the Jewellery

Unless directed by a medical professional. The jewellery keeps the channel open and allows pus to drain. Removing it can trap infection inside, which is significantly worse.

How to Prevent

Standard infection prevention: quality jewellery, gentle saline cleaning, no touching, no twisting. Treat any developing infection signs (increasing pain, hot swelling, pus, fever) early before they progress to abscess.

Telling the Types Apart

The Soft Test

Gently press the bump. Soft when pressed = likely irritation hypertrophy. Firm = likely hypertrophic or keloid scar. Tender and pus-filled = possible abscess.

The Time Test

Bumps that appear within the first 1 to 4 weeks and respond to addressing the cause are irritation hypertrophy. Bumps that persist after the cause is removed and the piercing has otherwise settled are likely scar tissue. Bumps that grow over weeks and extend beyond the wound area are keloid territory.

The Pain Test

Irritation bumps are mildly tender at most. Hypertrophic scars are usually painless. Keloids can be itchy or painful but not typically hot or severely tender. Abscesses are hot, severely tender and getting worse.

Step-by-Step: Dealing With a Bump

Step 1: Don’t Panic

The vast majority of bumps are minor and fixable. Take a breath and approach it systematically.

Step 2: Identify the Likely Cause

Check the jewellery (is it titanium or something cheaper?). Check your sleep position (have you been on the pierced side?). Check your aftercare (have you been using anything besides saline?). Check your habits (have you been touching, twisting or knocking the piercing?).

Step 3: Address the Cause

Swap to titanium. Set up sleep position properly. Simplify aftercare. Stop touching. Each of these on its own can fix many bumps.

Step 4: Continue Basic Aftercare

Sterile saline twice a day. No additional products, no specific bump treatments. The body fixes the bump once the cause is removed.

Step 5: Watch for Improvement

Improvement should begin within 1 to 2 weeks. The bump should fade within 2 to 4 weeks once the cause is genuinely addressed.

Step 6: Come In If No Improvement

If 4 to 6 weeks pass without improvement, the cause has not been identified or addressed correctly. A studio check can identify what is happening.

What Not to Use on Bumps

Tea Tree Oil

Common online recommendation. Tea tree oil is an irritant for many people and often makes bumps worse rather than better.

Apple Cider Vinegar

Highly acidic. Damages healing tissue. Makes bumps worse.

Aspirin Paste

Some online sources recommend crushed aspirin mixed with water as a poultice. Skin contact with aspirin causes irritation in many people and provides no benefit. Skip it.

Toothpaste

Contains abrasives and ingredients that irritate. Not appropriate for piercings.

Hydrogen Peroxide

Damages healing tissue alongside any bacteria. Slows the resolution of bumps.

Saline Soaks

Home-mixed sea salt soaks are unreliable. Pre-mixed sterile saline spray works for cleaning but should not be relied on as a bump treatment.

Warm Compresses (Sometimes)

Worth a note. Warm compresses can sometimes help reduce the size of irritation bumps by encouraging blood flow and helping the body reabsorb the tissue fluid. Use a clean cloth soaked in warm (not hot) water for 10 minutes, once or twice a day. This can help but does not replace addressing the underlying cause.

The bump is the symptom. The cause is what to address. Tea tree oil and aspirin paste are treating the symptom while the cause continues. Quality jewellery and protected sleep position treat the cause.

When Bumps Are Actually Something Else

Migration

Sometimes what looks like a bump is actually the jewellery starting to migrate towards the skin surface. The skin over the jewellery thins and a small raised area appears. The piercing is rejecting rather than developing a bump. See a piercer to assess.

Granuloma

A small fleshy growth that can sometimes form near piercings. Looks like a small smooth tag. Rare and usually responds to swapping jewellery and basic aftercare.

Cyst

A small fluid-filled sac that can form near piercings, particularly in oily skin areas. Different from an irritation bump. May need GP attention if persistent.

Pimple Adjacent to the Piercing

Common to mistake a regular pimple near a piercing for a piercing bump. If the bump is on regular skin near the piercing rather than at the piercing entry or exit, it may just be a normal pimple.
