Septum Piercing Healing
How septum piercings heal, the realistic timeline, the aftercare specifics and how to manage common concerns through the months of healing.
Septum piercings heal in 4 to 6 weeks for the surface and 6 to 8 months for full internal healing. The pierced sweet spot (soft tissue notch between cartilage walls) heals more easily than dense cartilage. Sterile saline twice a day. Avoid touching, flipping the jewellery and blowing the nose hard for the first weeks. Quality jewellery throughout. Most septum piercings settle into easy long-term wear.
Septum piercings are some of the more forgiving facial piercings to heal because the location is somewhat protected and the tissue typically responds well.
Septum piercings have grown enormously in popularity over recent years. They have the advantage of being relatively easy to flip up out of sight when needed and they offer a wide range of jewellery options once healed. This page covers what to expect during the healing window and how to manage each stage.
What Gets Pierced
The Sweet Spot
The septum piercing typically goes through the soft tissue notch at the bottom of the septum, between the cartilage walls. This sweet spot heals more easily than dense cartilage.
If the Sweet Spot Is Pierced
The piercing is comfortable, heals reasonably quickly and produces a clean long-term piercing. This is the standard septum piercing.
If Dense Cartilage Is Pierced
The piercing is more painful at the time, slower to heal, and more prone to long-term issues. Always pierce the sweet spot when anatomy allows.
Anatomy Variation
Not everyone has a clearly defined sweet spot. The piercer assesses anatomy before piercing. Some people are not suitable candidates for septum piercings; honest assessment is essential.
Acute Phase
Mild swelling. Brief tenderness. Saline cleaning twice a day. Avoid touching and flipping the jewellery. The first week needs the most care.
Settling Phase
Surface healing completes around week 4 to 6. Internal channel matures over 6 to 8 months. Most things return to normal during this longer phase.
The Healing Timeline
Days 1 to 3
Mild swelling. The nose may feel slightly tender. Sneezing can be uncomfortable. Eye watering is normal during piercing and immediately after. Resolution within 24 to 48 hours.
Days 4 to 7
Swelling resolved. Tenderness on direct contact only. The piercing settles into its initial state. Crusting at the entry and exit points begins.
Weeks 2 to 4
Surface continues to heal. Crusting reduces. The piercing looks essentially normal externally. Cleaning routine continues twice a day.
Weeks 4 to 6
Surface healing complete. The piercing is comfortable. Minor tenderness still possible on direct contact. The first downsize may happen around this time if the initial bar was longer than the final size.
Months 2 to 6
Internal channel maturation. The piercing looks settled but is still developing internally. Avoid jewellery changes, swimming and other stress on the channel.
Months 6 to 8
Full healing. The channel has matured. Jewellery changes safe. Swimming safe. The piercing has reached its long-term state.
Aftercare Routine
Sterile Saline Spray
Twice a day, morning and evening. Aim the spray at the entry and exit points. Let it sit for 30 seconds. Pat dry with disposable paper towel.
Hands Off
The biggest aftercare rule for septum piercings. Avoid touching, flipping the jewellery or playing with it. The temptation is real because the piercing is right there in your field of vision, but each touch sets healing back.
No Flipping
Do not flip the jewellery up and down during healing. The flip motion drags through the channel. Wait until fully healed (around 6 months) before regular flipping.
No Nose Blowing
For the first week or two, avoid blowing your nose hard. The pressure stresses the fresh piercing. Gentle nose blowing is fine once the initial healing has settled.
Avoid
Strong nasal sprays during healing (talk to a doctor if you need one). Heavily fragranced face creams that drift into the nose. Picking at crusts inside or outside the nostrils.
Specific Septum Concerns
The Smell
Septum piercings can develop a specific smell from accumulated oils and skin cells. Normal. Cleaning and quality jewellery reduce it. Some smell is part of having a septum piercing long-term.
The Sneeze
Sneezing with a fresh septum piercing is briefly uncomfortable. Cover your mouth gently rather than letting the sneeze produce maximum force. Most clients adapt within days.
Sense of Smell
Should not be affected by a septum piercing. If you notice changes, it is more likely to be unrelated (cold, allergies) rather than the piercing.
Cold Weather
The metal in the jewellery can feel cold in winter. Quality titanium minimises this compared to other materials. Some clients find the cold uncomfortable during the first winter with the piercing.
The Flip Trick
Healed septum piercings with the right jewellery can be flipped up into the nostrils to hide the piercing. Useful for work or other situations. Wait until fully healed before relying on this.

Eating and Drinking
No Specific Modifications
Unlike oral piercings, septum piercings do not require dietary changes. Eat normally.
Hot Food
Steam from very hot food can briefly affect a fresh piercing. Let food cool slightly. Resolves within a few days.
Spicy Food
Can briefly stimulate nasal mucus production. Not a problem but worth being aware of in the first week.
Alcohol
Skip the first 24 hours. After that, moderate drinking is fine. Heavy drinking during healing extends the timeline.

Jewellery Considerations
Initial Jewellery
Usually a straight bar or a horseshoe-shaped retainer. The piercer chooses based on anatomy and what will be most comfortable during healing.
Material
Implant grade titanium for healing. The septum environment has occasional moisture from nasal mucus, so quality material matters.
Initial Gauge
Most septum piercings start at 16 or 14 gauge. Going to thicker gauges later requires stretching, which is its own process.
Style Change Timing
Wait until 6 months for any style change. First style changes are best done at the studio.
The Hoop Question
Hoops, captive bead rings and clickers are popular for healed septums. The angle of insertion is fiddly; first hoop change should be at the studio.
The Retainer Option
For work situations where the piercing must be hidden, smooth quartz or PTFE retainers sit invisibly in the channel.
The Flip Mechanism
One of the appeals of septum piercings is the ability to flip the jewellery up into the nostrils, hiding it.
How It Works
U-shaped or curved jewellery can be rotated upward so both ends sit inside the nostrils rather than below the nose. The result is essentially invisible from outside.
When to Start Flipping
Once fully healed (around 6 months). Flipping during healing drags the jewellery through the channel and causes setbacks.
The Practical Use
Useful for work environments that do not allow visible piercings, formal events, and similar situations where you want to hide the piercing temporarily.
How Long It Stays Hidden
You can wear the flipped position for hours or all day without issues once fully healed. Avoid sleeping in the flipped position long-term as the bar can press into the inner nose.
The Limitation
Not all septum jewellery flips. Hoops with decorative elements at the bottom do not flip cleanly. Plain horseshoes and certain hoops do.
Common Concerns
The Smell
Already mentioned but worth repeating. Some smell is normal. Cleaning and quality jewellery help. Stretched ear lobes have similar issues.
Crusts Inside the Nostril
Common during early healing. Do not pick them out aggressively. Soak with saline during cleaning. They soften and clear naturally.
Tenderness When You Bump It
Direct contact tenderness for the first weeks. Reduces over time as the piercing settles.
One Side Looks Different
Mild asymmetry in entry and exit appearance can be normal. Significant asymmetry warrants studio assessment.
The Jewellery Sits Off Centre
The piercing should be symmetric. If the jewellery looks visibly off centre, the piercing may have been placed asymmetrically. Studio assessment can determine whether this is a problem.
Allergic Looking Symptoms
Persistent redness, intense itching or rash around the piercing suggests jewellery allergy. Swap to verified implant grade titanium and symptoms typically resolve within a week or two.
Long-Term Septum Care
Cleaning
Normal hygiene during showers and face washing. The active saline routine ends once the piercing is fully healed.
Regular Jewellery Cleaning
Once a month, remove the jewellery and clean it thoroughly. Wash the nostril area gently. Reinsert clean jewellery.
Material Quality
Stick with implant grade titanium or solid 14k gold long-term. Avoid plated jewellery which wears and exposes base metal.
The Flip Habit
If you flip regularly, the channel can develop a slight bend over years. Not a problem aesthetically but worth being aware.
Stretching
Some clients stretch septum piercings up over time. Different process from lobe stretching. Discuss with the piercer before attempting.
Septum piercings are one of the more rewarding facial piercings to heal. The first week is the demanding bit, and after that the piercing largely takes care of itself. The hands-off rule is what matters most.

Common Mistakes
Flipping Too Early
The most common septum-specific mistake. Flipping the jewellery during the healing window drags it through the channel and causes bumps and setbacks.
Touching It Constantly
The piercing is right in your field of vision and very tempting to touch. Each touch adds bacteria and stresses the channel.
Blowing the Nose Hard
The pressure stresses fresh piercings. Gentle nose blowing only for the first weeks.
Picking Crusts From Inside the Nose
The crusts inside the nostril are easier to pick than external ones. Resist. They soften and clear with saline.
Using Decongestant Nasal Sprays
Strong nasal sprays can irritate healing piercings. Discuss with a doctor if you need them.
Changing Jewellery Early
Wait the full 6 months for style changes. The piercing might feel fine sooner but the channel is not ready.
If You Wear Glasses
Glasses do not affect septum piercings significantly. The frame sits above the piercing area. No specific modifications needed.
Sunglasses
Same as regular glasses. No issues.
Face Masks
Face masks pressing on the nostril area can occasionally press on septum jewellery during long wear. Choose a mask shape that does not press on the nose tip.

Septum healing is one specific path. The hub covers the broader topics of healing, jewellery, common issues and recovery.
The Honest Summary
Septum piercings heal well when the anatomy is suitable and the piercing goes through the sweet spot. The first week needs care: saline twice a day, hands off, gentle with the nose. After that, most of the healing happens invisibly over months. The reward is a versatile piercing with many jewellery options and the ability to hide it when needed.
Anatomy assessment matters. If a piercer says your anatomy does not suit a septum piercing, listen. Forcing a septum into unsuitable anatomy produces worse outcomes than choosing a different piercing entirely.
Septum piercings combine a relatively manageable healing window with one of the most versatile post-healing aesthetics in facial piercings. The ability to flip the jewellery hidden makes them practical for many lifestyles. Get the right anatomy assessment, follow simple aftercare and the piercing rewards you with years of comfortable wear and easy maintenance.
